Lastly, all names should be changed for confidentiality purposes.   July 23/2021 – date of conversation   History   Senior J finally got low incoming housing and was transitioned from a treatment centre that he partook for for one year. There was planning  for transition into low income housing. The goal was to get him assisted living as he does require some assistance as he has limited mobility. Due to limited resources at this time there is nothing available for him to move into.    Prior to going to the treatment centre he lived on the streets for about 12 years. He partook in various criminal activities and spent two of those years in custody. Upon leaving custody he returned to the streets.He has been clean from street drugs for one year and is currently on the methadone treatment. He has been diagnosed with bipolar and a general anxiety disorder with panic attacks. He currently is taking medication to address the symptoms. He seems to be happy about having his new home but is concerned that since he has difficulty with mobility and with minimal support he is concerned that he will not be able to do it on his own and is already having challenges. He does use a wheelchair and when he does work his way out into the community sometimes he just stays out there because he’s so tired.   Conversation TO GO INTO TRANSRIPT BELOW:     Basically the essence of the interview is that he doesn’t feel safe living on his own without support. At the building he is at they do wellness checks every couple of days but they actually don’t follow through on that. This causes him anxiety and stress because he worries that he may fall down or need help. He has one leg and sometimes he will use his other leg to prop himself up to get in and out of the shower but he is concerned that one day he will slip and nobody will know that he’s even hurt. He stated that there’s a lot of criminals that live in his building and sometimes he’s even scared to go out or come in the building especially when he sees activity happening that is criminal in nature. He says the police come around but there’s a lot that doesn’t get addressed and he’d rather not speak about it because he doesn’t want to be targeted or hurt. J doesn’t want to lose his home because it provides him shelter but really would like to transition into somewhere that would provide him more support and where he would feel safe. He did express it was better when he was high as he didn’t have to think about this stuff.    It appears that this client is high risk to relapses as he said he has been thinking about it a lot and hasn’t bought of it in a long time and that this concerns home also.
